age when Disability began.
Catastrophic Disabilities

100% of wages – maximum $10,000 per month for up to 30 months.  Based on inability to perform 3 or more Activities of Daily Living.

During the initial 12 months of Disability the plan pays an additional: 30% of the first $14,286, not to exceed $4,286 of Pre-Disability Earnings. Based on inability to perform 2 or more Activities of Daily Living.

Personal Leave Integration Benefit

After 60 calendar days, you may use 50% sick leave and receive a 50% benefit from the Plan or use 100% sick leave or 100% personal leave and receive $1,000 per month
