PLEASE NOTE: This
card may not be used for new enrollment.
The card should contain up to date information for the
insured member with the beneficiary change on the bottom of the
card.
Do not fill out the top of the card with the beneficiary's
personal information.
If you are unsure of your coverage, contact your department
director
or call CLEA at 800-832-7333.
Complete the card with the new information and
mail it to:
CLEA, P.O. Box 31, Martell, CA 95654-0031
Your beneficiary information will be updated as
soon as we receive your card.
If you do not have Adobe Acrobat Reader
installed on your system you may do any of
the following:

Visit the Adobe
home page
to download a more recent version, or versions for other
operating systems such as Macintosh or Unix .
- Call the CLEA administrators, California Administration
Insurance Services, Inc.,
toll-free at (800) 832-7333 and ask for a change of
beneficiary card. We will mail a new card to you for you to
complete and return.
